According to mothersdaycelebration.com (which I have not bothered the fact-check), the youngest mother in history was 5 years old from Lima, Peru. She gave birth to a 6.5 pound baby via C-section in 1939.

The highest number of recorded children born to one woman is 69. She gave birth to 16 pairs of twins, seven sets of triplets, and four sets of quadruplets, and 67 of them survived infancy.
